Medical Claim Forms
Please find the attached claim form for your completion and return to
OnePath Life (NZ) Ltd, Private Bag 92131, AMSC, Auckland 1142.
To help with the assessment of your claim, please provide the following:
- A copy of your referral letter from your GP to your Specialist
- A copy of your Specialist’s report
- An estimate for your procedure
- A copy of the ACC decline or acceptance letter if this is an ACC related injury.
Please complete the claim form and send directly to OnePath (Life) NZ Ltd with the above information as well as any further information that may be relevant.
Once OnePath have received your claim form they will contact you.
